Whenever you add a profile picture to Telegram, the old 1 is not deleted.  People who visit your profile will be able to see all the profile pictures you've uploaded.  Here it is how to delete aged profile pictures on Telegram.  You can always hide your photo on Telegram, but the solutions available are limited.

You can clearly show the profile picture to anyone or just your contacts.  There is no way to completely hide your profile picture from Telegram, your best guess is to get rid of the profile picture altogether.  If you really don't want to do that, you can delete the most mature profile picture right away.  This will ensure that only your most recent profile picture is clear on Telegram.

Open Telegram on your iPhone or iPad and tap on “Settings” located on the bottom bar.

Tap the avatar photo at the top.

Now select the "Edit" option button at the top right corner.

Tap your profile picture again.

You will now be able to see all the profile pictures you have uploaded.  Keep swiping right to see them all.  Now navigate to any image you want to delete and select the trash can icon located below the image.

Choose “Delete” to delete the image.

You can repeat these steps for all the images you want to delete.

The steps to delete old profile pictures are a little different on Android.  Open Telegram on your Android smartphone or tablet and tap the three-line menu icon in the upper left corner.

Select your profile photo.

You can now swipe right on the image to see all your old profile pictures.  When you find one you want to remove, tap the three-dot menu icon in the top right corner.

Select "Delete" from the drop-down menu.

For the second time, press “Delete” and that's it.

You can do the same for all the profile pictures you want to delete.
