Patients need to bring a driver’s license and insurance card to the appointment. Anyone receiving a second or third dose, must bring a COVID-19 Vaccination Record to be updated. Baptist Health Hardin is unable to replace lost cards. Appointments are not required, but those wanting to schedule a time can do so at scheduleyourvaccine.com. To find other locations where you can receive the vaccination, go to vaccines.gov.

• Only the Moderna vaccine booster will be administered Monday. The daily vaccine schedule is as follows:

• Only the first, second and third doses of the Pfizer vaccine will be administered Wednesday and Friday. Vaccinations are available at the clinic for persons ages 18 and older. Children younger than 18 should go to their primary care provider for vaccinations, according to a Baptist Health Hardin news release.
